A personal care power of attorney


This is a document used to appoint someone to attend to “personal care” decisions on your behalf including decisions about health care, nutrition, clothing, hygiene, safety and consent to medical treatment such as hospitalization, and surgery, in the event you are unable to make those decisions yourself.

A personal care power of attorney should not be confused with a Health Care Directive. The Directive is a document, separate from but best used in conjunction with a personal care power of attorney,  in which an individual ‘directs’ the attorney as to their last wishes regarding care and medical interventions in the event a situation arises from which there is no reasonable expectation of recovery.
